Establish what the records mean
Keep the report version, source-file versions, metric definitions, mapping rules and review cutoff. Record corrections separately from new events arriving later. Identify who approved a definition change and whether old periods were recalculated with the same rule.
A worked example
Fictional correction: version 1 counts twelve call events as inquiries. Version 2 groups explicit retries and reports nine inquiries. The change note says three events were linked to existing requests, identifies the mapping evidence and preserves the twelve-event activity total. It does not describe the lower inquiry count as a decline in demand.
Choose the next action
Use a compact change log with old value, new value, reason, evidence and affected outputs. Preserve the original export and make a new corrected artifact. When a metric definition changes, label comparisons as restated or not comparable. Do not quietly revise historical records to support a preferred conclusion.
Check whether it worked
Check that totals, charts, worksheets and narrative agree after the correction. Reopen a sample of affected records. At the next review, use the current version consistently and retain access to prior versions. Recurring corrections may reveal a collection or mapping problem that deserves a process fix.
A repeatable review order
- Version and cutoff. Record the evidence and mark any missing answer as unknown.
- Old versus new definition/value. Record the evidence and mark any missing answer as unknown.
- Evidence and reason. Record the evidence and mark any missing answer as unknown.
- Affected periods and artifacts. Record the evidence and mark any missing answer as unknown.
- Reviewer and reconciliation check. Record the evidence and mark any missing answer as unknown.
